TCM (Traditional Chinese Veterinary Medicine) at OHANA uses the Four Examinations — observation, listening, inquiry, and palpation — to identify your pet's constitutional pattern and design a targeted herb prescription. We combine this with modern diagnostics for a whole-system view of health.
This intake form guides our pre-visit diagnosis. By sharing your pet's symptoms, constitution, and daily habits in advance, we can identify patterns such as Qi deficiency, Blood stasis, Yin deficiency, or Phlegm-Damp accumulation from the very first session — and select the most appropriate formula accordingly.
